Replace Insurance Benefits

Replaces all insurance benefits under a coverage with the provided set.
Benefits not included in the request are deleted. This is a destructive operation.
Note: For some PMS systems (e.g., Eaglesoft), benefits are shared at the plan level and this operation
may affect all patients on the same plan.
